The Independent Physician Association of America (TIPAAA) Selects Chronic Care Management, LLC as Its Preferred CCM Provider

Parties anticipate wide scale CCM program participation among TIPAAA's 300,000 physician members

Chronic Care Management, LLC today announced that it has been selected as the preferred provider of chronic care management services for members of The Independent Physician Association of America (TIPAAA), the leading trade association serving independent and integrated physician associations in the U.S.   TIPAAA currently represents over 3,500 IPAs and provides education, advocacy, resources and success tools to its membership network that spans the country.

“We are thrilled to partner with TIPAAA and its IPA and physician members to help provide an outstanding chronic care management experience for the physicians that this forward thinking and dynamic organization represents,” said Dr. William Mills, President and Chief Executive Officer of Chronic Care Management.  Dr. Mills said, “As a practicing physician myself who understands the daily challenges independent practices face, I created our CCM platform to be practice friendly.   We provide several options ranging from software only to full service which can include staff and medication management support --- tailored to the specific needs of our client practices and the patients they serve.”

"We have heard from our members that they want to participate in CCM, but due to the multiple requirements, most have not begun their CCM program. We are confident that our new partnership with Chronic Care Management and Dr. Mills will accelerate CCM participation among our membership."

Albert Holloway, MBA, MHA, President & CEO, TIPAAA

“We are excited to partner with Chronic Care Management to offer our members and the patients they care for an elegant CCM solution,” said TIPAAA President and Chief Executive Officer Albert Holloway, MBA, MHA. “We have heard from our members that they want to participate in CCM, but due to the multiple requirements, most have not begun their CCM program. We are confident that our new partnership with Chronic Care Management and Dr. Mills will accelerate CCM participation among our membership.”

Although approximately 100 million people in the U.S. have multiple chronic conditions and would benefit from a chronic care management (CCM) program, participation rates have been low among physician practices thus far.  It has been reported that the slow national adoption is due in part  to physician concerns of inadequate reimbursement and disruption.  Dr. Mills noted, “When we partner with practices, we work to understand their current and staffing, and then provide a customized solution to help facilitate the practice’s CCM success, which typically leads to healthier patients and healthy bottom line margin increases.”  Mills continued,   “We are tremendously excited about empowering independent physicians with a clinical and financial tool to help drive goal-directed, portable care planning, alongside an important new revenue source.  We look forward to closely partnering with the and practices TIPAAA represents to help drive their clinical and financial success.”

Chronic Care Management’s cloud-based platform places each patient at the center of an interdisciplinary, connected care network called My Care which synchronizes patients and their health care providers around a comprehensive, portable care plan. Built to enable value-based care transformation and a patient-centered care management model, Chronic Care Management aims to work with select partners to provide practices and organizations of all types the “right fit” CCM solution.  Dr. Mills added, “We are looking forward to attending the upcoming 21st annual TIPAAA National Meeting in San Antonio, Texas March 3-5, 2016 and meeting many fellow TIPAAA members .”
