The Company, which provides chronic care management services to locations that serve approximately 200,000 chronically ill Medicare beneficiaries, is focused on adding value for patients and providers.

The Company has selected ExactCare® Pharmacy, LLC to be its primary medication management solutions provider. ExactCare, based in Cleveland, simplifies daily medication routines, promotes medication adherence and improves health care outcomes for patients struggling with chronic conditions and polypharmacy. ExactCare’s approach features custom multi-dose packaging that consolidates pill bottles into a single pack by combining doses based on administration times which is delivered to patients’ homes each month. The company’s comprehensive solution also includes medication reconciliation, ongoing refill management and the support of clinical pharmacists to ensure effective medication therapy.
